Our Community Service department belives in a patient advocacy structural approach to ensure we meet your healthcare needs and provide additional support upon your visit. These services are availble for our patients and uninsured children and adults. Our staff is comprised of professional health educators that are cultural linguistically competent to effectively assist the residents in East SAN JOSE

Our friendly and knowledgeable outreach and administrative staff are available to help patients or their loved ones wade through the choices. Patient Assistance Program (Low or No Cost Prescription)

We can enroll our patients to receive their prescriptions at low-cost or no-cost based upon their eligibility requirements.

Outreach as a Resource to the Community

Outreach is an effort by a dedicated team within in the FCHC organization to connect its services to the community we serve.

We market, educate, survey and provide critical information to our patient population as well as the community at large. The role of the team is not only to disseminate information, but to also provide engagement through delivery services such as Family PACT, and nutritional consulting.

The outreach teams strategy is tightly aligned with the organization's mission, and define targets, goals, and milestones.
