Foothill Community Health Center is dedicated to providing a safe and confidential space for patients to obtain support through the difficult transitions and challenges in life. It is our goal to offer compassion, education and resource referrals that will cultivate and promote healthy lifestyles of individuals and families in our community.

Our Behavioral and Mental Health Staff

Our counseling staff is committed to providing high quality, compassionate and linguistically and culturally appropriate services to the diverse community of East Side San Jose. The onsite counseling team includes a Licensed Psychologist and Marriage Family Therapist Interns who are supervised by a licensed MFT Supervisor.

Our counseling services include individual services for adults and children, family therapy and therapeutic groups at an affordable rate.

  • Life Transitions and issues related to aging

School Based Student Services

Starting October 2010, Foothill Community Health Center has partnered up with selected schools in the East Side Union High School district and the Alum Rock Union School District to provide on-site counseling services to their students at no cost to the patients. The FCHC School Based Mental Health Program offers skilled Marriage Family Therapist Interns to provide counseling services pertaining to peer-relation issues, self-esteem, academic issues, behavioral problems, social skills, divorce, grief and loss, LGBT youth support and family conflict. Our MFT Interns aim is to collaborate with school staff, families and the FCHC Outreach team to provide wrap around services and a unique plan of action to support the child’s needs.

 

Appointments

Please call our clinic at (408) 729-9700 to schedule an appointment with one of our mental health providers. It is important to schedule an appointment ahead of time as we are unable to accommodate walk-ins at our center. Currently, mental health services are available during the day and evenings. Since we are not a 24 crisis centers please contact 911 or a 24 crisis hotline such as (408) 279-3312 if you need urgent services.

Referrals

Foothill Community Health Center will provide referrals to appropriate agencies when patients are:

  • Acutely Suicidal
  • Potentially Violent/Homicidal
  • Suffering from severe psychiatric Illness

Counseling Service Fees

FCHC front staff will assess patients for Public Health Benefit Insurances or provide a sliding scale fee based on income, family size and need.
