Student Health & Wellness Program


Foothill Community Health Center’s unique Student Health & Wellness Program (SHWP) provides a nurturing approach to deliver high-quality behavioral health, medical treatment, dental care, and vision services to pre-kindergarten through 12th grade students at NO COST from the comfort and safety of a familiar school campus setting. The SHWP is a research-validated and innovative strategy of giving children life’s basic needs and tools for success.

PROGRAM GOALS

  1. To enhance student access to high quality medical, dental, vision, and behavioral health services and support underserved schools throughout Santa Clara County
  2. To partner with schools to enable successful social, physical health, and educational outcomes for at-risk youth and their families
  3. To promote longer healthier lives for the low-income student population we serve and protect the public health within the schools and surrounding community

SHWP SERVICES

Some of our services include but are not limited to:

  • Medical - Immunizations, Annual Check-up, Sports Physicals, Chronic Disease Management, Adolescent Health
  • Dental - Dental screenings and Treatment, Cleaning, Fillings, Crowns, X-rays
  • Vision - Vision Screenings, Eyeglasses Prescription, Comprehensive Eye Exam
  • Behavioral Health Counseling - Self-Esteem, Academic Problems, Social Skills
  • Children’s Health Coverage Enrollment Assistance

HOW TO ENROLL YOUR CHILD INTO THE SHWP

  • Parents and/or Guardians MUST complete and submit a SHWP Form to any of Our Health Centers (see below).

  • SHWP Forms are available in: English Spanish Vietnamese

  • For assistance or any questions regarding SHWP, please call (408)755-3904.

  • All Students and Children (up to 18 years old) enrolled into the SHWP may receive services at any of Our Health Centers. 




STUDENT HEALTH SCREENING AND IMMUNIZATIONS

To improve student health care access, we are also able to provide the necessary student health screenings and immunizations at your school campus:

  • Dental Screenings (Pre-K to 12th Grade)
  • Vision and Hearing Screenings
  • Scoliosis Exams (7th Grade Females & 8th Grade Males)
  • Immunizations (Pre-K to 12th Grade)

As a result of the student health screenings, we will also offer follow-up care services for students to receive on-going services in medical, dental, and/or optometry at our nearest clinic.
