Special Health Programs We Are Developing Through Our Foothill Health Center Program

CPSP (Comprehensive Perinatal Service Program)

This is a state certification program that provides at no cost comprehensive range of "perinatal" services to female patients that have been determined to be pregnant. The services include the full range of prenatal care up until the third-trimester (36 weeks) by a Foothill Medical Provider with arrangements made for hospital delivery with an obstetrician at the point of delivery as well as post-partum care for the new mom and her infant. The program would also offer an extensive range of support services that include: nutrition, psychosocial, mental health counseling, health counseling (including home visit), and health education programs (individual and groups).

Family PACT

This is a California State Health Program that provides comprehensive family planning coverage to patients including non-parental consent services for minor children at no cost. Services include general health evaluation, full range of contraceptive services, full service laboratory assessment for STD's, and the full range of reproductive evaluation, testing, screening, pharmaceuticals, and laboratory services. (including HIV Screening and Testing).

CHDP

Child Health Disability and Prevention services is a free services that is provided to patients under 19 years of age that includes a comprehensive health physical i.e., medical, dental, mental health, vision, and other health evaluations as indicated by the health provider.

IMMUNIZATION ASSISTANCE (Vaccines for Children)

This program provides the full range of required and recommended immunizations to children as required under the Center for Disease Control (CDC) at no cost to the family under any number of available public heath initiative programs.

GATEWAY

This is a federal and state program that provides immediate comprehensive services to potentially Medi-Cal eligible children up to 18 years of age at no cost on the basis of "presumptive eligibility" until such time that it is determined that they don't meet the qualifications for Medi-Cal or until they are provided Medi-Cal benefits.

PRESUMPTIVE ELIGIBILITY

This is a free program available to female patients that have had a positive pregnancy test and are "presumed to be eligible for Medi-Cal" wherewith the PE program kicks in during the first visit until the Medi-Cal benefits kicks in or it is determine that the patient is ineligible.

DIRECT RELIEF:

Foothill is a certified recipient to Direct Relief (DR) which provides varied donations (pharmaceuticals, health supplies, and medical instruments) at no cost to the safety net clinic members. The donations are dispensed on a monthly basis dependent on their inventory.

SLIDING FEE SCHEDULE

Low income uninsured patients may qualify for subsidized (part-pay) discount fees depending on their income and number of dependents, if they fall within 200% of the poverty rate as determined by the front-desk staff.

STATE LICENSING FOR THE MONTPELIER HEALTH CENTER

This is a pending State Application to secure a state medical and dental facility license from the State Department of Licensing and Certification for the Montpelier Health Center.

 

 

 

 

 

PENDING GRANTS AND CONTRACTS

The following is a current listing of various Foothill Health Center Grants, Contracts, and Programs that is presently pursuing:

HRSA Section 330 Community Health Grant. This is a federal funding grant through HRSA for $750,000 that provides funding to support the provisions of health care services to high risk vulnerable low income individuals and families in the Foothill Target Area.

HRSA School Based Capital Grant. This is a HRSA awarded School Based Capital Grant to plan and develop two high school based clinics (Mount Pleasant High School and the San Jose Conservation Corps High School in the amount of $277,000.

HRSA Capital Improvement Program is a Grant for $210,000 to provide resources for capital improvement programs for the Foothill Community Health Center.

HRSA Capital Development Construction and Equipment Grant. This is a $1.3 million application to HRSA to secure funding to built out and modernize the former county public clinic (Chaboya Public Health Center) in South San Jose.

Mini Grant of $1,000 awarded from the Health Trust to go towards the formal opening festivities of the opening of the Independence High School in the latter part of November 2011.

This is a pending grant from State Family Planning Council for $85,000 to plan, screen, educate high risk individuals for Chlamydia in the Foothill Health Center target area.

This is a pending grant from the State for $85,000 for the development of an educational program that focuses on HIV education, screening, testing, and treating individuals for the HIV communicable disease that targets the Foothill Community Health Center target area in East San Jose.

California State Public Health Grant is a nutritional education and outreach grant for $150,000 to provide comprehensive education nutritional services to high risk families and adolescents in the San Jose Foothill Clinic target area.

Pending application for Partnership to promote "Tobacco Free Santa Clara County" that focuses on youth and adult smokers from target area. Mini grant is up to $40,000.

Pending grant application from the Health Center Entry-Level Work Force that focuses on community health Centers that concentrates on programs that entail recruitment, training, and retention of entry level staff and allied health workers. Funding up to $200,000.
