The San Jose Foothill Family Community Clinic (now known as Foothill Community Health Center) began serving low-income families and individuals in East San Jose as a nonprofit clinic in August 1996. The clinic provided comprehensive-primary medical care to the community from its one Story Road Clinic.
In 2019, Foothill Community Health Center (new name and management character) provides medical, dental, vision, pediatrics, OB/GYN, chiropractic, podiatry, health & nutrition education, behavior health, public health insurance enrollment, chronic care management, and a health home program service through six school-based health clinics and ten neighborhood health centers.
We are a patient-oriented health center that aims to provide comprehensive health care to the Bay Area. Our mission intended to appeal to our diverse patient population and other community stakeholders. It represents our vision and mission for the future.

Carlos lived in Santa Clara County his entire life, attending middle school and high school in San Jose. His educational spectrum encompasses a Bachelors and Master's degrees in engineering, as well as a Law degree.
