Health care are changing faster and unpredictably than ever. Thus, the unification of Foothill Community Health Center and Tri-City Health Center entail incremental improvement on existing healthcare services.

The collaboration of both organization will provide a larger, more efficient, operational, financial and patient-focused services. As we will become the largest Federally Qualified Health Center in the Bay Area.

Such development and innovation is essential and critical for the on-going change of healthcare.

We are a fiscally responsible, independent, not-for-profit community health center and a key member of our community’s vital health safety-net. Who believes in the journey to health and wellness is a shared responsibility with each other and with our community.

State of the Organization

Combined the initial size will include:

- 20 Clinics

- 6 School-Based health Clinics

- 4 Mobile Clinics

- More than 600 employees

- 90,000 + individual patients

- 480,000 + patient visits

No reduction in staff is anticipated at either Foothill or Tri-City clinic sites to preserve the continuity of care for both FQHC
