Medi-Cal Enrollment Assistance

Medi-Cal is a program that provides free or low-cost health coverage for individuals or families with limited income or resource.  Medi-Cal covers low-income adults, families with children, seniors, persons with disabilities, pregnant women, children in foster care and former foster youth up to age 26.  Medi-Cal is offered to eligible Californian residents with limited income.  Health plans available through Medi-Cal offer a set of important benefits called essential health benefits.  These essential health benefits fit into the following 10 categories:

o    Ambulatory patient services.

o    Emergency services

o    Hospitalization

o    Maternity and newborn care.

o    Mental Health and substance use disorder services, including behavioral health treatment.

o    Prescription drugs.

o    Rehabilitative and habilitative services and devices.

o    Laboratory services.

o    Preventive and wellness services and chronic diseases management.

o    Pediatric services, including dental and vision care.

You can apply online on A single application will let you know if you qualify for coverage through Covered California or Medi-Cal.  You can also apply in person at your local county human services agency or by phone by calling Covered California at 1-(800)- 300-1506.  Once the application is submitted, it will be sent to your local county human services agency for a determination if you are qualified for Medi-Cal.  If more information is needed, the county will contact you to provide more documentation.  During the next 45 days, the county will mail you a notice telling you if you qualify for Medi-Cal.  If you are eligible, you will receive a Medi-Cal benefits identification card (BIC) in the mail and also an information package that explains the available Medi-Cal health plan options in your county and how to enroll.  Our Health Program Enrollment Specialist (HPES) are here to assist any individual or family in applying/ renewing for healthcare coverage through Medi-Cal.

340B Prescription Program

As a patient of Foothill Community Health Center, you are eligible for a discount on prescribed medications that can save you 20% to 50% on the cost of your medication.

Foothill has contracted with a national pharmacy chain and a local independent pharmacy to offer convenient locations for your prescriptions to be filled and picked-up. Your FCHC Primary Care Physician can send an electronic prescription request to its contracted pharmacies or give you a hand-written prescription for you to take to the contracted pharmacy.

Medical Prescription

Comprehensive Perinatal Service Program (CPSP)

The program offers a wide range of services to pregnant Medi-Cal recipients from the date of conception through 60 days after the month of delivery.

Services include: nutrition counseling, psychosocial counseling, mental health counseling, health counseling (including home visits), and health education programs (individual and groups).

Birthing Centre

Family PACT (Planning, Access, Care, Treatment)

Family PACT services are designed to support contraceptive methods for women and men, as gender appropriate, by assisting individuals who have a medical necessity for family planning services.

Secondarily, Family PACT includes assistance with family planning-related services to achieve and maintain optimal reproductive health.

There are two categories of services available in the program: Family planning services and family planning-related services for specified reproductive health conditions.

Birth Control Treatment

Every Woman Counts (EWC)

EWC is a comprehensive, public health program that assists uninsured and underinsured individuals in obtaining high-quality cancer screening and follow-up service.

In addition to offering screening and diagnostic services, the program is designed to facilitate regular rescreening of women with normal of benign breast and/or cervical conditions to provide diagnostic services for individuals presented with symptoms and/or abnormal screening results and to refer for treatment when necessary.

CHDP Gateway

Gateway provides immediate comprehensive services to potentially Medi-Cal eligible children under 18 years old at no cost on the basis of "presumptive eligibility" until such time that it is determined that they don't meet the qualifications for Medi-Cal or until they are provided Medi-Cal benefits.
